What are some extreme metal bands/albums using different instruments?

By different instruments I mean, things which are quite rare on the particular metal. Not piano/keyboards/violin because one can find any of these in every group of 5 bands. Let alone guitar/bass/drums =p
Plus, I'd love to hear black or death metal with accordion. But extreme folk with accordion is already overused. Basically just ask yourself "is this instrument used broadly in the genre or is it innovative?" If it's the latter, then it fits here.

By bands using I mean they play that instrument often, like in almost every song, or along an entire album at least, and not only they used that particular instrument in a cover song on a live version of a mini CD in 1996.

Some examples to ilustrate:

Sigh has a sax in the line-up

Sear Bliss has a trombone

Somnolence is cello-driven

It's okay if it's not exactly the instrument per se, but a (very good) sinth imitation too.
So... Share your knowledge =]

Combat Astronomy sounds like what you're looking for. Saxophone, trombone, trumpets and other stuff are often part of the deal.

Metallica played the hurdy gurdy, lol

Not sure if it qualifies as "extreme" but Ihsahn has used a horn section on Erimita. I'd broadly categorize it as black metal, but it's almost jazz, really.

I would like to hear small children grim vocals, that would be pretty evil sounding!

Sockweb's vocalist is 9 years old. =D
Not sure if it qualifies as "grim vocals" though... But perhaps you'll like it.

Grayceon is one of my favorites here. They use the cello throughout this album and its one of my all time favorite prog metal albums with symphonic elements.
